How does it work?

Your data will be stored on your local machine using JavaScript and cookies. It is not stored on a host somewhere. This introduces some issues you need to be aware of.

1. You should use Forms Assistant from your personal computer.

   You should not use the Forms Assistant in a public cluster. Since your personal data is stored on the PC you are using this form from you will not gain anything by using it in a public cluster. Anything you fill in here will be available to the next user of this PC. Although your name and email address are not all that personal you s hould be aware of this.

2. Rebuilding your machine will erase this data.

   This data is stored on your computer in a file called cookies.txt. If you delete this file you will need to come back and fill in this form again.

3. This data will remain for 180 days.

   This data is stored on your computer for a period of 180 days.

4. How do you update your information?

   Fields are updated by returning to this form and typing over the old values. Fields may also be deleted by clearing the fields below.
